New Patients

To establish care with a Primary Care Provider at Cedar Point Health, follow the simple steps below.

Step 1: Find a Provider

Review the list below to find a provider who is currently accepting new patients. Visit the Provider Bios page to learn more.

Step 2: Sign Up for Patient Portal

Access our new patient portal HERE 

Step 3: Message our Administrative Department

Once you are registered with the portal, send a message to the Administrative Department requesting the full new patient paperwork and policy agreements.

Step 4: Fill Out & Return

Staff will respond with the attachment for you to complete. Once completed, send us back the paperwork via the same platform it was received or drop off to any of our front desks.

Step 5: Get Ready for Our Message

Our team will contact you to schedule your Establish Care appointment within 3 business days of receiving paperwork.
